If you are registering with a Residential Contract, please review Board Policy # 5152 for additional requirements.

Description of Contact Preferences
- Emergency/Priority - if you uncheck this checkbox, you exclude this method of contact from receiving emergency messages
- Attendance - if you uncheck this checkbox, you exclude this method of contact from receiving attendance related messages
- General - if you uncheck this checkbox, you exclude this method of contact from receiving general school messages, such as those sent by the principal or District
- Teacher - if you uncheck this checkbox, you will exclude this method of contact from receiving messages sent by teachers via Campus Messenger
- PTC/PTA - if you uncheck this checkbox, the District won't share this method of contact with the PT Council or PTA

In an emergency, if no parent/guardian can be contacted, the District will contact the following Emergency Contacts in the order of the contact sequence preference set by the parent/guardian.
Proper identification will be required before a student is released to emergency contacts.
Please do not add parents/guardians as emergency contacts. In case of an emergency, including early dismissal, parents/guardians will be contacted first. Only if no parent/guardian can be reached, will your child's Emergency Contact(s) be called.
Please enter at least one phone number for each emergency contact.

The District is responsible for the town's Census. For Census purposes, please list here all individuals who reside at the Primary Household address other than:
- parents/guardians,
- school-age children,
- preschool students who have been referred to the District for special education services.
Please enter parents/guardians on the Parent/Guardian tab and please enter all children (school-age, as well a preschool children who have been referred to the District for special education services) on the Student tab.

Student cell phone numbers and email addresses are optional fields. However, the District encourages parents/guardians to provide these, especially for high school students.
