Register Your Sponsor

1. You must submit a signed Registration Signature Form to the YELLOW BOX in the SENIOR OPTIONS OFFICE (room 165) By Friday, March 7th, 2008. Only then will you receive an E-Mail with the link to register ONLINE. That e-mail will give you instructions and access to this required registration.

2. The online “SURVEY MONKEY” Registration will provide us with information about you, your sponsor, and your site, that will be necessary to approve your project. You must completely and accurately answer ALL questions before you can write a proposal.

3. SEE THE SAMPLE REGISTRATION FORM TO SEE WHAT YOU NEED .

4. To gather basic information about your Sponsor, download and, with your sponsor, fill out the Sponsor Information Form from the Forms section.

5. FOLLOW THE DIRECTIONS. YOU WILL NOT GAIN ACCESS TO THE ONLINE PROPOSAL SITE UNTIL THE REGISTRATION SIGNATURE FORM IS RETURNED AND YOUR SPONSOR REGISTRATION IS ACCURATELY COMPLETED AND SUBMITTED ON SURVEY MONKEY.

6. Following the successful completion of these tasks, you will be sent another E-Mail instructing you to how to access and write your proposal online.
