Please take a moment to comply with this very important request.
Several of the New York State mandated screenings, take place in your pediatricians office, at the well child visit. Please be certain to send in your child's annual examination, each and every year.
In addition, 7th and 8th grade students who want to participate in our modified sports program will require a recent physical on file (use Health Certificate Form only).
ALL reports should be mailed, emailed to lfaneca@scarsdaleschools.org or faxed to 722-2850. (We are open all summer, and can receive faxes 24/7.)
